Sona: Clinical notes
doctors can trust
Healthcare product design, built around trust.

The product
Sona is an ambient scribe: it listens to a patient visit and drafts the doctor’s note. The doctor reviews it, makes changes, and signs, with every step designed around trust.
Sona is a placeholder name used for this project.
The problem
For doctors
Hours of typing every day, and notes that are hard to trust when it’s unclear what the tool wrote or where it came from.
For patients
A doctor facing a screen instead of them, and details that get missed.
How it works
01 · Recording
Questions while the patient is still here
Gaps in a note usually show up after the patient has left. Sona suggests follow-up questions during the visit, like a missing medication dose or a symptom worth checking, answered with one tap.

02 · Notes
A note the doctor can trust
Anything Sona wrote is always marked. Suggestions wait for the doctor’s approval, and signing unlocks only after every section has been reviewed.

03 · Transcript
The source behind every line
See who said what, and play back any detail in the note from the exact moment it was said.

04 · Insights
Suggestions, not decisions
Possible diagnoses, treatments, tests, care gaps, and clinical trials, each with its reasoning and sources. The doctor decides what to act on.

05 · Billing codes
Codes with the evidence attached
Each suggested code shows the note sections and recording moments behind it, and mismatches are flagged before the note is signed.

06 · Color system
Every color has one job
Violet always means Sona wrote it. Red is only for safety, like allergies. Yellow means something needs a look, and green means it’s done. At a glance, the doctor knows what’s theirs, what needs attention, and what’s finished.

The experience
Doctors spend less time typing and more time with their patients. Gaps get filled before the patient leaves, every detail can be traced to where it came from, and nothing gets signed without review. The result is a note doctors can confidently put their name on.
